Provide Case Management for students.
* Assess students to determine eligibility for services.
* Maintains appropriate student records of services provided.
* Make home visits to provide services.
* Provide parents and teachers with intervention ideas.
* Work with other agencies as needed to meet students’ needs.
* Excellent written and verbal communication skills.
* Develop and follow Individual Family Service Plans or IEP’s.
* Assist families in finding and accessing resources as needed.
* Collaborating with other professional support staff.
* Participates in staffing and other school meetings.
* Performs other related duties as assigned.
Requirements
* Bachelors degree and license as an ECSE teacher.
* Possess and maintain a valid Minnesota driver's license.
* Knowledge of the development sequences for children.
* Knowledge of how to use standardized assessment tools.
* Knowledge of Special Education Due Process procedures.
* The ability to work with students, staff, and parents in a variety of environments.
* Ability to lift students weighing up to 50 lbs.
* Ability to use a computer.
* Provide own vehicle for home visits.
