School Social Worker, 1.0 FTE, Middle School East and Middle School West
Serves as a member of a multidisciplinary team to assess and/or reassess students referred for special education services. Prepares and implements appropriate service plans for special education students, refers child/family to community agencies as needed, documents progress in the Individual Education Plan (IEP), attends IEP meetings. Provides individual and group support to help students resolve and/or cope with personal, emotional and social problems that interfere with their educational experience. Provides services to parents as an integral part of the task of helping students; to gain their parental participation in resolving their child’s problems. Assists families to better understand the school and its’ programs and to strengthen the communication link between school and home. Assists teachers and school administration through identification of special needs a child may have in school. Serve as a liaison between home, school community and other agencies through maintenance of positive relationships. Provides crisis intervention services. Participates in faculty meetings, special education meetings and inservice training. Fosters social work development through participation in workshops and conferences. Performs specific duties related to school social work which may be assigned by the Executive Director of Student Support Services. Performs other duties of a similar nature or level.
