Job Description:
- The goal of the PLUS Case Coordinator/ Social Worker is to improve eligible students’ attendance and increase graduation rates through a network of educational programming, family and children’s mental health supports, and correctional and social service supports all in a school-based setting.
- The plus case coordinator is responsible for the day-to-day oversight and management of assisting enrolled Eligible people to achieve success in reaching educational goals.
- The plus case coordinator assists with problem-solving, school participation, attendance, and behavioral concerns as well as the coordination of other supportive mental/chemical health referrals.
- The plus case coordinator will be assigned to work in classrooms with eligible students to assure individual goals are achieved.
- The plus case coordinator will consult with educational team members during professional learning community meetings, participate in re-admit/IEP meetings, meet with outside agencies, court officials, probation officers, mental/chemical health supports, Minnesota Visiting Nurse Agency (MVNA), and parents to facilitate transition into mainstream education settings as appropriate.
- The plus case coordinator will provide access to college & career planning by connecting with the school counselors to access post-secondary training, employment, and employment training.
- Work is performed under the general direction of the Principal.
Essential Duties and Responsibilities:
- Promote culturally and linguistically responsive practices, assist teachers to maintain a positive learning environment, and reinforce classroom learning and district expectations
- Assist teachers and staff in educational functions and other services to maintain a safe, diverse, and productive learning environment
- Work with students in classrooms to reinforce classroom learning and improve socialization and personal skills (case coordinator will rotate classrooms throughout the week)
- Refine and adapt various educational service systems designed to meet the needs of students
- Create a safe and predictable environment for students by supervising students during passing and other unsupervised areas (cafeteria and passing time)
- Set high expectations for students while supervising and monitoring
- Promote respect, de-escalate confrontations, and enforce BCCS policies and rules of conduct
- Daily record data for attendance, academic and behavioral assessments; maintain records of student progress and activities to assure all required written reports are completed and submitted to all agencies
- Cultivate relationships and sustain successful collaborations with the local community and government organizations
- Provide targeted instruction to students for structured learning opportunities
- Implement and monitor student progress to provide assessment, program services, and evaluations
- Facilitates non-biased assessments, provide interpreters and cultural liaison for meetings and provides translated forms
- Meet with outside agencies, court officials, probation officers, mental/chemical health supports, Minnesota
- Visiting Nurses Agency (MVNA) and parents to facilitate transition into mainstream education settings as appropriate
- Collaborate with administrators, counselors, other community representatives to develop a case plan for an eligible student
- Connect eligible students and families to outside services offered to eligible persons and families without interrupting the learning environment to:
- Address goal planning and other needs(academic, social, emotional, physical, financial, etc.)
- Include action steps and follow updates to ensure Eligible Person receives services and meet their goals
- Receive signature of parents or guardian required for Eligible Persons under 18 for field trips, school media receiving mental health, dental, vision, access to college & career planning, post-secondary training, employment, and employment training by connecting with school counselor, etc. except if court-ordered
- Review every three months to see if the eligible person is achieving the goal and record in QuickBase corresponding to the date of contact and/or activity and method of service indicated (face-to-face, email, text, social media, other)
- The plus case coordinator will participate in professional learning community meetings, re-admit/IEP meetings, meet with outside agencies, court officials, probation officers, mental/chemical health supports, Hennepin County Nurse and parents to communicate student academic, behavior, and attendance progress
- Communicates with parents, students, and staff to convey pertinent information while maintaining the confidentiality of personal information; answers questions and explains school rules
- The plus case coordinator will maintain files and keep records of student progress and activities to enhance basic skills The plus case coordinator will help eligible students solve personal and family issues by communicating with specific programs, local community resources and regional community services (it is highly recommended that students are empowered not enabled) Communicate with administrators, teachers, school personnel and students in a tactful manner to resolve issues and conflicts
- The plus case coordinator will mediate disputes
- The plus case coordinator will assure all required written reports are completed and submitted to the administrator and other agencies monthly
- To increase school stability for at-risk students, students experiencing homelessness and determined highly mobile by McKinney-Vento Act, the plus case coordinator will:
- Communicate regularly with transportation coordinator, school administrator, district homeless liaison, parent, and student of changes with transportation services
- Establish procedures with students and parents to receive information about student transportation needs and pickup location
- Provide parent and student with the contact information of transportation and other go to staff members in case plus coordinator is unavailable
- Develop a routine system for students and families to communicate with the driver
- The plus coordinator will transport as needed under the direction of the school administrator
- Other duties as apparent or assigned
Qualifications:
- Bachelor’s degree in human service or social work
- Bilingual (highly recommended)/ willing to learn Spanish
- Experience working with high need high school aged students
