A. General Purpose
The Prevention Partnerships for Healthy Youth (PPHY) Grant Coordinator, herein referred to as the Grant Coordinator, is responsible for overseeing the general development of a community plan to address youth substance use prevention among Black, Indigenous, and People of Color (BIPOC) youth within the Faribault Public Schools community. The Grant Coordinator is responsible for coordination of coalition meetings, implementation of initiatives, and assisting with reporting requirements for funders. Other duties include coordinating with lead culturally specific organizations to recruit youth and family participation, as well planning and implementing substance use prevention communication campaigns.
The Grant Coordinator can expect support from the Minnesota Regional Prevention Coordinators, Minnesota Department of Human Services ATOD Grant Supervisors, the PCN Coordinator Peer Group and Faribault Public Schools in the performance of duties as requested.
This position requires some travel. Flexible hours are required for day and evening/weekend duties as needed. Additional duties as assigned by the MN DHS-Behavioral Health Division.
Duties and Responsibilities
1. Community Organizing
Engage lead culturally specific organizations to foster participation and ownership in achieving prevention goals
Identify key community members as part of assessing readiness/needs, assuring diversity in community participation
Identify prevention needs and resources by collecting relevant information
Collaborate with the Rice County Chemical and Mental Health Coalition to coordinate action team and achieve goals
Educate community members to develop and maintain community capacity by ongoing mentoring and training of community members
2. Planning and Evaluation
Assess community readiness and needs by collecting the most current local data through established assessment methods
Implement strategies tailored to the needs of the local population in consultation with community/coalition partners.
3. Education and Skill Development
Work with community partners to select and deliver culturally appropriate education and skill development activities
Educate others so they are able to present accurate, appropriate information about substance use prevention
Disseminate appropriate educational/informational materials to community members and stakeholders
Complete Substance Abuse Prevention Skills Training (SAPST) and Prevention Ethics to obtain Certified Prevention Professional (CPP) certification during the 1st year of employment and maintain certification in the remaining years of the grant.
This is a grant-funded position, through a grant received by Faribault Public Schools from the Minnesota Department of Human Services. The grant runs through September 30, 2025.
