School Social Worker
Full time position available. School Social Worker’s license required. Essential Functions: Promotes academic success by reducing social, emotional, economic and environmental factors that interfere with learning or other barriers to school success. Works closely with educational teams on developing appropriate interventions for classroom success. Plans, organizes and runs individual and group social skills sessions. Conducts interviews to obtain social history information. Prepares reports about assessment interviews. Investigates issues which surface through referrals. Conducts home visits as necessary during assessments and investigations. Reviews student records. Refers students to and serves as a liaison with outside agencies such as the social services bureau, juvenile probation, juvenile detention and the juvenile court. Counsels with students and parents concerning living and learning environments. Works with outside agencies to assist schools in creating plans related to school attendance. Assists in diagnosing students’ special needs. Provides crisis intervention and prevention for families and students. Works with families in crisis to connect with outside resources for food, shelter and transportation. Collaborates with community agencies and industry professionals to meet individual student needs. Description: Special Education Teacher – at West Junior High. This position is for the remainder of the 16/17 school year. Job Summary: Special Education Teacher to work with a variety of disabilities. Deliver direct & indirect services, monitor IEP progress on goals & objectives. Follow due process and IEP procedures & paperwork, communicate and work collaboratively with families & staff.
Licensed School Nurse
Description: Licensed School Nurse. Position works 1.0 FTE (40 hours per week) for 186 days per year following teacher’s schedule. This positon will be at the Tokata Learning Center, but also has District Wide responsibility should the need arise. Job Summary: Assess individual student health needs; prepare health plans, delegate nursing and medical functions to other staff as appropriate for the task, individual student and staff person. Administer and document medications following district procedures. Maintain health records. Obtain immunization records for all students and complete state immunization reports. Perform nursing procedures such as tracheostomy care, gastrostomy care, diabetes management, and nebulization. Participate in the special education team, completing the health portion of special education evaluations & Individual Education plans, supervising PCAs and completing Medical Assistance billing procedures. The LSN works with the part-time LPN to assure staffing for health office and that a nurse accompanies students with nursing needs on field trips. The RN participates in regular LSN meetings and other projects/ committee representation as needed.

POSITION SUMMARY: Provide academic and goal-based learning experiences that enhance student progress across all settings. PRIMARY JOB FUNCTIONS: 1. Contribute to and participate in the building student support teams. 2. Serve as an IEP/IFSP case manager. 3. Work as a team member in staffings. Facilitate team meetings to review student progress and develop IEP/IFSP as related to student needs. 4. Plan and implement appropriate educational programs for individuals. 5. Implement educational plans as describe on individual IEP/IIIP/IFSP and provide information for building staff as needed. 6. Maintain appropriate records and reports. Collect data on progress on goals and objectives for each student and meet all required timelines. 7. Develop, coordinate, and evaluate transition related experiences for students, if applicable. 8. Provide work direction to paraprofessional staff members. 9. Communicate effectively with students, families, educators, and administrators regarding student programs and progress. 10. Collaborate with students, families, related service providers, outside agencies, and other team members to promote growth of each student. 11. Work cooperatively with general education teachers and staff and provide building consultation, support and training as needed. 12. Create and maintain a professional working relationship with families and staff. 13. Obtain and demonstrate knowledge of general education curriculum including the grade level outcomes and state standards. 14. Conduct assessments, comprehensive report results, and follow Minnesota state criteria in area(s) of special education eligibility as well as mandated timelines. 15. Determine and complete purchase orders for supplies, materials, and equipment based on student needs. 16. Participate in building and District level professional development activities. 17. Understand and maintain boundaries and confidentiality with students, families and staff. 18. Perform all other duties and responsibilities in certification areas as directed by the Director of Special Services and/or the Building Principal. 19. Other duties as assigned.
