Provide Case Management for students.
* Assess students to determine eligibility for services.
* Maintains appropriate student records of services provided.
* Make home visits to provide services.
* Provide parents and teachers with intervention ideas.
* Work with other agencies as needed to meet students’ needs.
* Excellent written and verbal communication skills.
* Develop and follow Individual Family Service Plans or IEP’s.
* Assist families in finding and accessing resources as needed.
* Collaborating with other professional support staff.
* Participates in staffing and other school meetings.
* Performs other related duties as assigned.
